The Sophie Prize

The nomination form should be sent to the Sophie Foundation no later than November 1^st.

THE FOLLOWING INFORMATION IS REQUIRED:

About the nominee

1. Name, full address, phone, fax, e-mail, web site (if there exists one).
   

2. Curriculum vitae (for nomination of individuals)

3. A “punchline” highlighting in a few words why your candidate deserves the prize (approximately 75 words).

4. Summary of the nominees work and achievements and some thoughts about how the Sophie Prize could effect the candidates future work (no more than 400 words)

5. 1 - 3 persons (references) who are familiar with the nominees work.

About the Nominator

2. Short description of the relationship between nominator and nominee (no more than 100 words)

3. A short description of nominators work/organisation/institution (no more than 100 words)
